Click on a link to download an application or a registration packet. Complete and send in an application with the appropriate fee to enroll in one of SPP's school sessions. You will be contacted to confirm your spot and meet with the Director. After meeting with the Director, complete all the forms in the SPP registration packet and return them to SPP with the appropriate fees. Forms are also available for pick-up at the school.

Additional Instructions for Registration Packet
Use the below licensing office name, address, and telephone number to fill out the top portions of the forms LIC 613A (Personal Rights) and LIC 995 (Child Care Center Notification of Parent's Rights). Please note that you keep the top portions of those forms.
